Under The Weather Form

This form must be completely filled out and shown to the cashier upon pick up.

The person who is allowing someone else to pick up their meal must fill out and sign this form. This person also authorizes Dining Services to swipe one meal from their meal plan.

The person picking up the meal MUST have their own WM ID with them to match the pick-up person identified on form.

This form is only good for one meal. A separate form must be turned in for each meal pick-up.
